Patient Responsibilities

Therapy requires your active participation, both in the clinic and at home.
- Promptness is critical for optimal care. Lateness may result in rescheduling.
- Notification of cancellation is requested at least 24 hours prior to scheduled appointment.

Two missed or a total of 3 cancelled/missed appointments will result in notification of discharge to you, your physician and insurance as appropriate. A new referral from your physician will be required prior to scheduling any additional appointments.
