• Insurance card
• Referral from physician
• All necessary authorizations
• The patient is responsible for co-pays at time of service. Additional charges that may not be covered include, but are not limited to, purchase or rental of equipment and supplies. Medicare and most private insurance companies do not pay for exercise equipment. Cash, Checks and Major credit cards accepted.
